The most common type of ASD, the ostium secundum defect, is caused by a shortened valve of the foramen ovale (foraminal flap) resulting from excessive resorption of the septum primum or to deficient growth of the septum secundum (Fig. 4–3). Figure 4–3 Ostium secundum ASD. A, …

Secundum atrial septal defect (ASD), located in the central part of the atrial septum within the floor of the fossa ovalis, is the most common type of ASD. It is a shunt between the systemic and pulmonary circulations. Patients with an isolated ASD often remain asymptomatic early in life. Secundum ASD or ostium secundum atrial septal defect In this case, using cardiac CT the size of the ASD could be accurately measured (1.7 cm). During respiratory inspiration, the negative intrathoracic pressure causes increased blood return into the right side of the heart. The most common type of ASD, the ostium secundum ASD, generally occurs in isolation. It is located centrally in the atrial septum, superimposed on the fossa ovalis. The ostium primum ASD is the second most common type.

MATERIALS AND METHODS: Sixty-five children (mean age, 5.4 years ± 2.7 [SD]) with ASD and inconclusive TTE results Ostium Secundum is a hole that is found within the flap of tissue (septum primum) that will eventually close the hole between the two atria after birth. With either type of ASD, ASD will usually cause the blood flow from the right atrium to skip going to the right ventricle and instead flow to the left atrium. Secundum ASD o 50-70% of ASDs o Isolated defect o Twice as common in females o Simple mid-septum ASD Secundum ASD, same hole as PFO but hole is not covered by septum secundum Hole too big or septum secundum insufficient Results from arrested growth of secundum septum or excessive absorption of primum septum Secundum ASD. The secundum defect is the most common form of ASD and generally occurs as an isolated defect.

According to these data, development of PH in patients with ASD is frequent and seems to be related to the diameter of the defect. However, patients with smaller and simpler defects may also develop PH. This seems to occur at older ages and not to be gender-dependent.

An ASD is characterized by a defect in the interatrial septum that allows pulmonary venous return to pass from the left to the right atrium (i.e., left-to-right shunt), resulting in right atrial and right ventricular chamber dilation. There are four types of ASD. The first type is an ostium secundum defect.

Atrial septal defects (ASDs) can be classified as morphologic defects of the interatrial septum that directly communicate the right and left atria (ostium primum ASD, endocardial cushion defects, ostium secundum ASD) and as defects that do not involve the interatrial septum but physiologically behave like interatrial septal defects (sinus venosus ASD, unroofed coronary sinus).

Best visualized on subcostal four-chamber view of the heart.

patent foramen ovale (PFO) vs. atrial septal defect (ASD) PFOs and ASDs differ etiologically and anatomically . a PFO results from the failure of fusion of the flap valve of the normal foramen ovale, which is formed by the overlapping septum primum and septum secundum

This type of ASD is a true deficiency of the interatrial septum and is distinct from the frequently identified patent foramen ovale. Best visualized on subcostal four-chamber view of the heart. Ostium secundum ASD's appear as a larger than expected area of dropout in the central portion of the septum secundum (in the vicinity of the foramen ovale) or as a deficient foraminal flap (septum primum) that fails to cover the entire foramen ovale.
